Requirements

• Current, unrestricted occupational therapist licensure
• Current CPR certification
• Valid driver’s license, personal auto insurance meeting state minimums or specified limits, and reliable transportation
• Preferred: Minimum six months of occupational therapy experience
• Preferred: Home health experience
